Its a Panasonic 4200uf (Low impedance - much harder to find) 10 volt Power cap. It comes with a surface mount support, that can easily be removed by bending the legs up. Its dimensions are 17X19mm without the support.

I paid $3.12 through Digikey for it (had to buy 5 at a time hence selling extras).
.90 cents for the original shipping divided.
Plus another 1.50 now to send it (with padded envelope price).
