Here's the Layout
Insert this layout gif into a word doc and resize it to:
W=1.36"
H=2.23"
Then get a sheet of labels, peel off all the labels and print you doc onto the shiny side of the sheet with a laser jet printer. Then you'll be able to transfer the circuit onto a blank PCB with an iron. Notice the image is inverted.
